Subject: Health; chronic care; insurance; reimbursement
Statement of purpose: This bill proposes to improve insurance reimbursements to health care professionals for chronic care and chronic care management to ensure that health care professionals are adequately reimbursed for encouraging patients to manage their chronic illnesses.
AN ACT RELATING TO REIMBURSEMENT FOR CHRONIC CARE
It is hereby enacted by the General Assembly of the State of Vermont:
Sec. 1. CHRONIC CARE REIMBURSEMENT
(a) The Blueprint for Health executive committee shall ensure that the plan for developing reimbursement systems aligned with the goal of managing the care for individuals with or at risk for chronic conditions in order to improve outcomes and the quality of care is implemented through a pilot project no later than July 1, 2007.
(b) The plan shall require that:
(1) reimbursement for counseling patients with chronic disease done by health care professionals shall be billed and reimbursed by time spent counseling; and
(2) health care professionals managing chronic conditions shall be reimbursed on a per capita basis for the number of individuals with chronic conditions treated by the professional in order to reimburse for services which are not subject to a billing code.
